FAKTA om hjärnskadade:
* Hjärnskador Ca 70.000 personer drabbas varje år av förvärvade hjärnskador, av olika grad.
* Konsekvenserna av en hjärnskada varierar oerhört mycket beroende på vilken typ av hjärnskada det gäller. Hjärnskadans omfattning beror på vart den sitter och alla komplikationer som kan uppstå i samband med hjärnskadan och inte minst vilken akutbehandling och vidare rehabilitering som ges till den skallskadade.
* Eftersom hjärnan styr alla våra sinnesintryck, gör alla våra tolkningar och bearbetningar, skapar alla våra tankar, bilder och föreställningar, styr alla muskler och övriga organ i kroppen. Med tanke på detta kan i stort sätt vilken funktion i kroppen bli handikappad som helst, när en hjärnskada inträffar.
Man delar in funktionsstörningar i fyra olika delar:
* Perceptuella funktionsstörningar:
Detta innebär att varseblivningen av omvärlden eller kroppens inre miljö, blir störd till följd av skador på ledningsbanorna, ett eller flera av de tolkningsområden i hjärnan som tar emot. Det kan vara rubbningar vad det gäller syn-, hörsel-. balansrubbningar. Det kan också finnas störningar på smak- och lukt, störningar i upplevelser av smärta, beröring, vibration, tryck och temperatur, A-törningar i kroppsuppfattning och upplevelsen av olika kroppsrörelser eller störningar i förmågan att integrera intryck från olika sinnen.
* Motoriska funktionsstörningar:
Förlamningar, apraxier, ataxier, dyskinesier, dysartrier mm.
Det vill säga kraftiga försvagningar eller störningar i förmågan att integrera intryck från olika sinnen.
* Kognitiva funktionsstörningar:
I denna kategori räknar man in störningar av det intellektuella förmåga, dessa störningar är för många "osynliga "för omgivningen. Men samtidigt ofta starkt handikapp för den drabbade. Hit hör språkstörningar, uppmärksamhets- och koncentrationsstörningar, olika typer av minnes- och inlärningsstörningar, störningar på vakenhet och initiativförmåga, störningar i förmågan till planering och brister i insikt och omdöme.
* Emotionella funktionsstörningar:
Störningar såsom förändrat känsloläge eller störningar i att kontrollera känslor och impulser. Dessa funktioner kan påverka beteendet och ger då omgivningen upplevelsen av e stark förändrad personlighet. Tex. aggressivitet, manier, hämningslöshet, förlorad impulskontroll, känslomässig avflackning eller förlorad självkänsla.
* Detta är den generella skadebilden, varje person är ju unik så det är olika funktionsnedsättningar och problem till följd av varje skada.
* Rehabilitering:
Rehabiliteringen på en hjärnskadad person är mycket långvarig och mödosam process. Hjärnans förmåga att bilda nya nervceller till hjärnan är mycket begränsad, tills bara för ett par år sedan trodde man inte att kroppen inte alls kunde nybilda hjärnceller. Fast nyare forskning har visat att kroppen kan nybilda nya celler i en viss del av hjärnan. Däremot finns det en rad olika mekanismer som gör att den skadade kan få tillbaka nedsatta funktioner eller märkbart förbättras.
Hjärnan har stor plasticitet (formbarhet) som gör ett framsteg i rehabilitering - förloppet kan ske under hela livet, även många år efter en hjärnskada.
* Reorganisation
När en del av hjärnan tar över vissa delar av det som är skadat på andra delar av hjärnan, och tar över dess funktioner. Hjärnan bygger delvis om sin struktur för att kunna fungera mer ändamålsenligt för de uppgifter den har att lösa. En total reorganisation kan inte ske, utan det utan det kan endast ske av de områden som har anknytning till det skadade området. Dessa kan då ta över eller ge stöd i vissa skadade delar av en funktion.
* Synapsbildning
En nybildning av nervceller kan endast göras i en ytterst begränsad omfattning, vet man. Däremot kan nya nervceller skapa nya förbindelser (synapser) med andra nervceller. Oskadade synapser kan bygga ut sitt synapsnät så att en viss kompensation för det skadade funktionerna kan ske genom ett utvidgat samarbetsnäst mellan oskadade nervcellsområden.
* Nyinlärning
Vissa nedsatta funktioner, kan fås tillbaks så att det nästan inte märks att man har en skadad cell. Detta sker i sådana fall genom att den skadade personen har lärt om sig alla de förlorade funktionerna på nytt. Många gånger får dessa funktioner läras in på ett annorlunda sätt än vad man har fått lära in de tidigare, för att kringgå de skadade funktionsnivåerna.
* Kompensatoriska strategier
Genom att lära sig nya strategier för att lösa problem kan vissa funktionsbortfall kompenseras så att det går att bli mer självständig trots sin hjärnskada.
* Automatisering
Genom en väl avvägd träning kan funktionella program nå en högre grad av automatisering, vilket gör att en mindre del av hjärnans kapacitet tas i anspråk och istället kan utnyttjas för simultana aktiviteter. Genom att till exempel träna upp motoriken kan större energi och resurser utnyttjas för till exempel perceptuella och kognitiva funktioner, energi och resurser som annars kanske skulle behövas bara för att hålla upp kroppen.
* Rehabilitering
Kräver kunnig och erfaren personal som kan ge en allsidig och rik stimulans och skapar de rätta förutsättningarna för individens rehabilitering utifrån, beträffande utgångspunkten. Rehabilitering kräver utöver en egen stark vilja, det krävs även ett starkt känslomässigt engagemang från ens omgivning, vilket är den mest avgörande drivkraften för den skadade personen, för dess återgång tillett självständigt och värdigt liv.

Jag vill sända ett stort tack till främst mina båda föräldrar och syskon, som jag har det mesta att tacka för att jag hade så pass bra rehabilitering som jag fick, både undertiden när den vanliga rehabiliteringsdagen var slut och när jag väl hade ledighet, samt när jag väl hade slutat på Rehab. De gav aldrig upp förrän jag var helt återställd, massor av tid, kärlek och ömhet om mig.
Jag vill även tacka alla kamrater som var hos mig så gott som varje dag och gjorde mig sällskap, de var också till stor hjälp för min rehabilitering tillbaka till det liv som jag numrera lever.
De sista jag inte får glömma att tacka är övriga kompisar jag hade innan min olycka som inte svek mig när jag förolyckades, utan nästan mer tvärtom, de ställde upp helt suveränt även de.
Ett jättestort tack till alla som har hjälpt mig tillbaka till ett värdigt levande, som jag har idag.
Epilog:
Det finns ännu kvar problem, som är beroende på denna olycka som jag har varit med om, och som gör att jag absolut inte är samma människa som jag skulle ha varit om denna olycka aldrig hade inträffat. Det går inte att upptäcka om jag sitter eller står framför dig, men om du ser att jag går iväg kan man lätt se att jag inte går perfekt utan slänger med mitt högra ben. Som alltså är en skada jag har kvar sedan olyckan.
Även mitt minne kunde varit bättre, men jag har fått lära mig att skriva upp allt som jag måste komma ihåg.
Har även ont i ryggen numera, nästan alltid.
Bara exemplet på vad jag jobba inom, som idag på ett dagcenter. Som det absolut inte hade varit annars, jag
hade jobbat som elektriker på byggen troligen.
Jag kommer gärna ut på skolor och dyligt och föreläser.

Fredagen den trettonde blev en verklig olycksdag för Martin Karlsson. Han krockade med tåget när han provkörde sin moped. Miraklöst nog överlevde han, men vägen tillbaka har varit lång.
Det har hunnit gå nitton år sedan olyckan som förändrade hela Martins liv. Livet därefter blev inte alls som han hade tänkt sig. Men han är tacksam att han överhvudtaget lever.
- Jag borde ligga ett par meter under jorden. Den resan jag har gjort tillbaka skulle inte gå att göra egentligen. Det är ett under att jag lever egentligen, säger Martin.
Visst har han men efter olyckan. Hjärnskadan gör att han har problem med minnet, blir mycket trött och har svårigheter med balansen. En nerv i högra benet skadades också och gör att han haltar när han går och han har dessutom ständigt ont i ryggen.
Låt oss ta det från början. Hela historien börjar redan sommaren 1991. Martin var 14 år och fick en nedmonterad moped av sin storasysters pojkvän. Hela sommarlovet monterade, skruvade och putsade han på sin kära moppe. I slutet av oktober skulle han fylla 15 och den födelsedagen såg han fram emot mer något annat. Redan i september var mopeden klar. Då var det förstås självklart att Martin ville provköra lite. Familjen bodde i Månsarp och hade ganska nära till ett skogsområde. Han tänkte att det vore säkrast att köra i skogen. Där fanns ju verken någon trafik eller några poliser. Hans lillebror hjälpte gladerligen att knuffa igång moppen och så åkte Martin iväg.
Efter ett tag korsade skogsvägen järnvägen, vid en obevakad övergång. Martin verken såg eller hörde tåget som närmade sig, utan for rakt över spåret. Lokföraren signalerade och tvärnitade, men att få stopp på ett tåg tar tid. Mopedens bakhjul träffades och Martin flög 25 meter i luften och hamnade på huvudet en bit ifrån rälsen.
- Utan hjälmen hade jag aldrig suttit här idag, säger han.
Räddningstjänsten var snabbt på plats. När utryckningsfordomen med blåljus och sirener kör rakt ner i skogen blev förstås folk i Månsarp väldigt nyfikna. De rusade ut och följde efter för att se vad som hade hänt. Martins lillebror och mamma sprang också, med de hade ingen tanke på att det hade hänt Martin något. Brodern som sprang snabbast, kom först fram till olycksplatsen och fick syn på den välbekanta mopeden. Tvärnitade vände han igen och sprang tillbaka mot sin mamma. En brandman stoppade honom och frågade honom om det var någon han kände. "Det är min bror", skrek han.
En granne tog hand om lillebror, medan mamma fick följa med Martin till sjukhuset.
En stund senare kom Martins pappa hemcyklade från jobbet, lyckligt ovetande om vad som har hänt. Han blev förstås förånad när han fick se att det står en polisbil på garageuppfarten framför huset. Polisen berättar att det har hänt en olycka och skjutsade honom till Ryhov.
När Martin anlände med amblans till akuten mötte han först en läkare som snabbt konstaterade att det rörde sig om skallskador, vilket inte var hans specialitet. Läkare nummer två undersökte Martin och insåg att skadorna var allt för komplicerade för vårdpersonalen i Jönköping. Martin behövde köras till specialisterna i Linköping. På väg ut mot ambulansen passerade de den första läkaren igen. Han upptäckte att Martins mage hade svullnat upp. Istället för ambulansfärd bar det av till operation. Magen öppnades och det visade sig att hela magsäcken var spräckt.
- Jag behövde opereras direkt, för jag skulle aldrig ha överlevt färden till Linköping annars, berättar Martin.
Väl framme i Linköping vårdades han i resperator, hade övervakning dygnet runt och hur många påsar dropp som helst. Läget var kritiskt, men föräldrarna fick inget svar på hur allvarligt skadad deras son var.
- En läkare sa till dem att de kunde vara glada om jag ens blev ett paket, säger Martin.
Själv minns han ingenting, vare sig av själva olyckan eller av tiden efter. Han tillbringade många månader på sjukhus, först i Linköping och sedan i Jönköping.
- Jag var som en bebis igen. Jag hade blöja, blev matad och kunde inte gå, säger han.
Men mot alla odds blev han sakta men säkert bättre igen. På rehbeliteringen fick han både träna fysiskt med sjukgymnasterna och mentalt med arbetsterapeuterna. Även när han var hemma hos sina föräldrar fortsatte de att träna med honom. På lördagarna brukade de hyra skolans gymnastiksal och vara där och träna.
- Min familj har varit helt fantastiska. Utan dem hade det aldrig gått, säger han.
Det sägs att nöden prövar vänner. Martins vänner klarade provet. De fanns ständigt vid hans sida. Trots att han fick gå om hela nionde klass och fick nya klasskamrater fungerade det hela tiden bra.
Första april året därpå fick han känna på hur det var att vara ensam. Då tillbringade han nästan en månad på ett rehabeliteringscenter i värmlandska Torsby. Hans lillebror har i sina dagboksanteckningar skrivit att Martin satt i rullstol när han åkte, men gick på kryckor när han kom hem.
Även Martins mamma har skrivit dagbok efter olyckan. Där finns allt samlat av vården, känslor och tankarna. Martin har allt detta i ett kompendium. Det händer att han föreläser om vad som har drabbat honom på olika skolor. Eleverna brukar sitta knäpptysta och lyssna på hans berättelse. Det han framförallt vill få dem att förstå är hur viktigt det är att använda hjälm, även när man cyklar.
- Jag kan berättelsen om mig väldigt bra. Men eftersom jag inte minns någonting själv känns det ofta som om jag berättar om någon annan, säger Martin.
Ända sedan han var liten hade han drömt om att bli eletriker och jobba på byggen. Den drömmen gick om intet i och med olyckan. Tiden på sjukhus väckte istället hans intresse för vården. Efter grundskolan sökte han sig därför till omvårdnadsprogrammet på gymnasiet. Redan på sin första arbetsplats mötte han kvinnan i sitt liv. Idag är de gifta och har en liten dotter. Till vardags arbetar Martin som vårdare för handikappade.
- Jag hade mycket väl själv kunnat vara en av dem, säger han.
Visst händer det att han tänker på hur livet hade sett ut om olyckan inte hade hänt. Och frågan "varför just jag" bär han förstås på. Den brukar han ha som sin utgångspunkt för sina föreläsningar, då han berättar om sin resa tillbaka till livet.

- Jag landade på huvudet, därav min hjärnskada, berättar Martin.
Tipsa en vänDet var den 13 september 1991, Martin var 14 år gammal och skulle fylla 15 i oktober månad. Han hade samma dag haft sin sista dag på praktiken som gymnastiklärare. Martin hade under sommaren själv monterat och målat en moped som han var väldigt upprymd över. Den hade blivit färdig kvällen innan.
- Året jag blev femton pratade alla om mopeder. Så jag var snabbt ute med min bror efter middagen för att kunna testköra den, berättar Martin euforiskt.
Brodern puttade igång mopeden som först inte ville starta och drog iväg genom skogen där han brukade springa.
Därefter gick allt väldigt snabbt. Martin kommer fram till järnvägsspåret där det saknas skyltar och där skog skymmer all möjlig sikt.
- Jag var väl medveten om att där fanns ett spår, men trodde att jag skulle hinna över, berättar Martin.
Flera ton mötte ynka kilo
Det gjorde han också, men inte mopeden. Tåget klippte bakdäcket med sina 47 ton vilket medförde att den 55 kilo lätta pojken flög flera meter bort och landade på huvudet. Chauförren som inte såg honom förren det var för sent tvärnitade och stannade efter 150 meter.
- Vore det inte för hjälmen och min fysik så hade jag inte suttit här idag. Jag landade på huvudet, därav min hjärnskada, och bröt inga ben otroligt nog, säger Martin Karlsson.
Det kommande halvåret minns han ingenting av. Läkarna på Ryhov i Jönköping konstaterade att det var en skallskada och skickade honom vidare till specialisterna i Linköping. Men det är inte förens nu som en av läkarna upptäcker att Martins mage är svullen och att magsäcken spruckit.
- Jag hade aldrig överlevt till Linköping med spräckt magsäck, säger Martin.
Efter två veckor i Linköping med övervakning dygnet runt och flera olika droppåsar, flyttades Martin tillbaka till Ryhov i Jönköping.
- Ni ska vara glada om han blir liggande och inget kan göra, ska en läkare på Ryhov ha sagt till Martins mamma.
"Känns som att jag berättar om någon annan"
Under hela återhämtningsperioden skrev hans mamma anteckningar, text som idag är hela grunden för Martins uppfattning av det förlorade halvåret.
- De berättade för min mamma att jag inte skulle minnas den här tiden senare, därför började hon föra dagboksanteckningar. Det känns därför som att jag berättar om någon annan, säger Martin.
- Jag var en bebis på 14-15 år. Ingen som helst kommunikation och hela tiden iförd blöja. Nya människor blev nya människor för varje gång de kom in, berättar Martin.
Onda Bengt
En av de som började arbeta med Martins balans var sjukgymnasten Bengt.
- Han är en av dem som jag faktiskt kommer ihåg såhär i efterhand. Onda Bengt som egentligen inte var så ond, bara jag som förknippade honom med smärta och träning, säger Martin.
Efter ett träningsläger och flera rehabbesök började Martin bli bättre. Vännerna hade hunnit gå ut nian och Martin fick gå om samma årskurs igen.
- Jag åkte buss som vem som helst. Fördelen med en liten by är ju att vi alla känner varandra. Så jag kände redan alla i min nya klass, berättar Martin.
Yrket inte längre aktuellt
Efter högstadiet valde Martin att gå omvårdnadsprogrammet då yrket som sjukgymnast krävde allt mer av hans betyg.
- Jag ville bli elektriker, en sådan som är ute på fältet, men det var inte längre aktuellt, berättade Martin Karlsson.
Senare fixade Arbetsförmedlingen en plats åt Martin på Torggården där han arbetade med handikappade i fem år. Det var även där han träffade sin nuvarande fru. Martin söker därefter jobb och hamnar på Majvallen där han är än idag.
- Jag borde ha varit där själv. Det finns en kille där som är precis som jag. Jag hade tur som var så ung och kunde rehabiliteras, säger Martin.
Håller i föreläsningar
Idag bor Martin Karlsson i centrala Jönköping med fru och ett barn. Han har problem med minnet, sin balans och har på senare tid även fått problem med ryggen. Martin kan inte heller cykla, men får köra bil med lite fler extra steg inför utbildningen.
- Jag håller även i föreläsningar och skriver via min blogg för att få ut hur viktigt det är med stödet från sina vänner och familj. Hur det är att leva med en hjärnskada, berättar Martin.
Järnvägsspåret i Månsarp är idag lika illa som för 20 år sedan. Det finns inga skyltar och lider av dålig sikt.
- Det är någonting som jag verkligen vill ha förändring på, säger Martin.

Hej, jag skulle vilja skriva stort tack till din mycket lärande blogg. Allt står uppstaplat om olika typer av nedsättning och hjärnans roll, allt som händer i den och varför. Min dotter var med om en olycka för 6 månader sedan. Mycket traumatiskt, men hon hade tur ambulansen var framme på 4 minuter. Koma inre blödningar och ja . inga ord. Efter 5 dygn sa de att hon överlever. Min dotter har kämpat så hårt och jag har försökt stötta med allt jag har. Det var samma sak med matning, blöjor etc. Men det hände nåt, hennes blåmärken försvann på 1 vecka, hennes inre bara hela. Nu vill alla läkare se henne och undersöka för till och med hålen i hjärnan märks inte , hon går talar, springer, studerar, läser, ja allt som förut. Liten skillnad i humöret det är allt. Och hon var högt högt på glascowskalan mellan 10-11. Både du och hon är ett mirakel.
